I had one of those. it was stupid heavy but virtually indestructible. This is mine. It's 20+ years old (I've had it for the past 12) and was made by a company called Zieman (they used to build some good quality trailers here in So.Cal.). It's steel, weighs about 80 pounds and hauls bikes great! I added the LED light strip since most of the time whatever bike I'm hauling blocks the tailights. The LED strip also has amber turn strips as well plus since I sometimes use this same HH on my front reciever, I leave the strip unplugged and no one can see it.
as far as the bike falling off the rack: that sucks, bad. sorry about that, thats terrible..
however, thats a tie down issue. always tie the hooks to the eyelet with the excess strap. loop around the hook with the tail of the strap and single knot it back so it cant come unhooked. my guess is that a hook came off. I've hauled bikes for well over 20 years on carriers, trailers, etc. Never lost one myself. even have had a trailer airborne with an aprilia falco on the back and stay secured. (my ex wife was freaked out because it was hers! lol).
